Today, we're thrilled to launch the Avora Template Library, a completely free collection of 262 dental chart note templates, meticulously organized by CDT code. Each template includes detailed notes, common denial triggers, and practical examples to streamline your documentation.
We covered it all: diagnostic, preventive, restorative, endodontics, periodontics, prosthodontics (fixed/removable implants), oral surgery, orthodontics, adjunctive services, and even 27 special workflow templates for procedures that don't neatly match a single CDT code (such as Invisalign visits, implant follow-ups, scans, retainer deliveries, and more).
Why we created these templates
Templates are the backbone of dental documentation. They shape how thoroughly visits are recorded, how accurately services get billed, and ultimately, how confidently charts hold up under audit scrutiny. Yet most practices today rely on templates that were copied from another office, edited once, and rarely updated, even as ADA guidelines and payer rules evolved.
We kept hearing the same questions from our dental partners:
- "What does a robust and defensible D2950 core buildup note really need?" Ever since the 2018 ADA revision, many offices have struggled to confidently document this highly audited code.
- "How clearly should we document D0150 versus D0120 or D0180?" The distinctions matter, especially since oral cancer screenings became mandatory for D0150 in 2021.
- "D4341, D4342, D4346: which is correct, and what's essential in each note?" Despite attempts at clarity, confusion around these perio codes persists.
- "Our templates don't reflect recent FDA guidance on amalgam restorations. How can we fix that?"
Proper dental documentation is complex, blending clinical details, insurance requirements, and legal necessities. We believed it deserved to be addressed comprehensively and transparently.
What's included in each template
Every one of the 262 pages serves as a practical reference, offering much more than just a note structure:
- Ready-to-use template text, complete with
[Field]placeholders, adapted from notes currently in use at real practices. - Clear usage guidelines, including ADA descriptors, decision-making logic for similar codes, and insights into typical audit checkpoints.
- Detailed documentation requirements, written clearly to ensure anyone reviewing the note later understands exactly what happened.
- Common denial reasons, highlighting the main reasons codes get denied or downgraded by insurance companies.
- Practical examples, demonstrating exactly how a real visit might look using the provided template.
- Related templates, helping you efficiently review entire procedure categories.
The entire library is searchable by code or name, grouped intuitively by category to help you quickly find exactly what you need.
A starting point, not a substitute for your judgment
These templates are reference samples and starting points, not clinical, billing, or legal advice. Before using any template in a patient chart, review it for clinical accuracy, your jurisdiction's documentation requirements, and your practice's medico-legal standards. Frequency rules, payer policies, and same-day billing logic vary by carrier, plan, and CDT revision. Verify against the patient's benefits and the most current ADA CDT manual. Avora makes no representation that these templates meet any specific clinical, legal, or insurance standard.
We made these templates free because we believe better documentation helps everyone. It reduces friction, improves accuracy, and gives clinicians valuable time back each day.
Where Avora fits in
Templates lay the foundation, but actually completing detailed notes for every patient, every day. That's the time-consuming part.
Avora transforms documentation by listening to your visits and filling out your templates automatically, including timestamped citations linked directly to your conversations. Many practices are already using these templates directly with Avora to further simplify their daily workload.
